Description:
The Manager of Community Health & Well-Being (CHWB) works in collaboration with the hospital and/or regional leadership to ensure the hospital and/or region is fulfilling its commitment to promote optimal health for those populations who are poor and vulnerable in the communities we serve by connecting social and clinical care, addressing social needs, dismantling systemic racism and reducing health inequities. This position supports strategy development, designs interventions, operationalizes the strategy, leverages resources and partnerships focusing on those who are poor and vulnerable, and addresses social needs and the social influencers of health. This position provides oversight of community health and well-being programs and colleagues and acts as a liaison to community agencies in assessing and addressing community needs.
Position responsibilities:
Knows, understands, incorporates and demonstrates the Trinity Health Mission, Vision and Values in behaviors, practices and decisions.
Manages and provides direction to the CHWB colleagues, operational and financial activities over one or more hospitals, inclusive of Community Health Workers.
Responsible for making decisions and managing risk that impact the functional area or program operations.
Oversees the work schedules and assignments, provides coaching and mentoring, as well as performance management to the CHWB colleagues. Ensures compliance with the hospital or regional personnel policies when operating the department.
Serves as a convener of internal and external stakeholders, representing Ministrys commitments to community health improvement and partnerships. Provides timely and regular updates to hospital leadership and any relevant board committees, particularly as it pertains to CHWB strategy, CHNA needs and strategies to address community needs.
Develops long- and short-term goals, objectives, plans and budgets or forecasts for CHWB programs to ensure continued quality of services.
Supports development and monitoring of an annual CHWB budget or forecasting (inclusive of community benefit) that aligns annual spending plans with the Ministrys goals and objectives while ensuring CHNA implementation efforts are resourced.
Serves as a leader for Health Ministry’s Social Care programs. Develops community-based referral partnerships and serves on community advisory committees to address patient social needs and the social influencers of health.
Supports the development and implementation of a Community Health Needs Assessment (CHNA) every three years in partnership with community stakeholders, placing particular emphasis on the needs of those who are poor and vulnerable and addressing social influencers of health.
Develops measurable community health improvement plans and implementation strategies that align with the community health needs assessment (CHNA) and Trinity Health strategic plan.
Designs, coordinates and promotes community-based interventions that address community needs and supports implementation strategy plan as part of the CHNA.
Represents hospital and/or region at professional or community organization meetings and on committees at local, state and national level in order to assist in developing new programs and policies for improved delivery of health services those who are poor and vulnerable.
Supports strategies regarding Anchor Institutions (i.e. local hiring and local sourcing) and policy, systems and environmental change initiatives in collaboration with internal and external partners.
Assures that appropriate or required program documentation is maintained for accurate reporting of the programs for community benefit compliance and/or state and federal grant appliance. Assures requirements for data collection include meaningful assessments of the programs provided to the community.
Maintains a working knowledge of applicable Federal, State and local laws and regulations, the Trinity Health Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.
Requirements:
Must possess a comprehensive knowledge of health disparities, social influencers of health, uncompensated and undercompensated care, community benefit, community and coalition building, public policy development and advocacy, and population health management normally obtained through a Bachelors degree in Health Care Administration, Public Health, Community Health, Social Work or a related field with five (5) years of strategic planning and fiscal management experience in community health and well-being area in a complex organization.
An equivalent combination of education and experience will be considered.
Ability to effectively manage and facilitate colleagues within assigned unit/department through strong leadership, communication and negotiation skills.
Ability to influence results, garner support and tactfully manage complex relationships and influences within and across the organization.
Ability to read the subtle nuances of situations and react/plan accordingly.
Ability to negotiate with peer level director and department heads on issues and programs that impact assigned unit/department.
Ability to build relationships and facilitate partnership agreements with all levels of the organization system-wide around health-related programs.
Excellent organizational, advanced analytical, presentation, interpersonal, and action-oriented skills. Demonstrated decision making skills.
Excellent written and verbal communication skills, including strong listening and comprehension skills, and excellent human relations and interpersonal skills.
Excellent program design and evaluation skills.
Superior attention to detail and ability to multi-task in a highly dynamic environment while managing multiple team member workloads to meet deadlines. Ability to adapt to a rapidly changing environment.
Must be comfortable operating in both a collaborative, shared leadership environment, and can work independently.
