Job Description:

M-F; On call rotation covering evenings, nights, weekends, and holidays

The Hospice RN provides a full range of hospice and palliative nursing care to patients in their homes, assisted living facilities, extended care facilities, hospital, etc. The RN must be able to work without supervision on a daily basis, manage his/her own schedule, and provide the best end-of-life care. The Hospice RN works in an interdisciplinary care setting to teach, assist, and support patient/family with the dying process. He/she identifies patient and family needs and provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care and Hospice policies and procedures. The RN must also provide palliative support to patients and families including pain/symptom management, consultation, assistance with creating a life plan, referrals to community resource, and education on disease progression.


Evaluates patient referrals to determine patient eligibility and status

Completes holistic, age-specific admission nursing history and physical assessment for all assigned patients.

Identifies and addresses comfort care needs.

Documents patient’s level of pain and other symptoms and identifies most effective methods of symptoms management.

Identifies psychosocial and spiritual needs of the patient and makes appropriate referrals.

Anticipates, prevents, and treats undesirable symptoms or secondary symptoms.

Sets priorities and revises own schedule as patients’ needs change.

Communicates changes in patient status to other members of the Interdisciplinary team.

Delegates to team members based on patient needs, continuity of care, staff skill and knowledge.

Minimum Qualifications


Graduate of an accredited school of nursing required

Current license as a Registered Nurse in the state of practice is required

Valid MT driver’s license and proof of automobile insurance required

Current BLS certification required


Bachelors degree of Nursing is preferred

Certification as appropriate for unit or service is preferred

Previous experience as a Registered Nurse is preferred

Physical Requirements:

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.- and -Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.- and -Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.- and -Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.- and -Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).- and -May be expected to stand in a stationary position for an extended period of time.
